Opportunity Description

HumanEdge has partnered with a medical facility in Wyocena, WI to hire an experienced Physical Therapist (PT) for a 12-week assignment – possibility to extend after the first 12-weeks.

Job Duties

  • 12-weeks 
  • ASAP start 
  • Guaranteed Hours 32, expected to work 40 
  • SNF setting with some outpatient and home-health. Ideally looking for 32-40 hours.

Experience & Skills Required

  • Master’s Degree in Physical Therapy
  • WI PT licensure required 
  • Must have excellent clinical and communication skills 
  • Demonstrates the ability to perform the following tasks without posing a threat to the health or safety of him/herself or others and without risk to self, co-workers, caregivers, or patient: lift, turn, move position and transport the patient into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. 
  • Sedentary tasks related to the job – approximately 10% 
  • Physical tasks as listed – approximately 90% 
  • Physical Therapist Performs evaluations and develops effective resident treatment plans to restore, maintain or prevent the decline of resident function, by planning and administering medically prescribed therapy treatments in accordance with federal, state and professional standards governing the facility.
  • Able to adapt quality therapy services and rehabilitation into a patient's home setting
  • Demonstrates competency in the use of modalities and equipment in the home care setting
  • Able to prioritize patient needs to achieve attainable treatment goals
  • Thorough knowledge of teaching/learning principles
  • Working knowledge of interdisciplinary care coordination
  • Operational knowledge of computerized clinical documentation and office application systems including the use of laptop/notebook computers. Familiar with Oasis Documentation
